UDSF is seeking a passionate Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in overseeing and coordinating services for older adults, individuals with disabilities, and veterans; ensuring they receive the support needed to lead independent and fulfilling lives.

 


Responsibilities:

Development of the person-centered service plan that address Participant identified areas of needs, goals, intervention, and progress of goals.

Coordinate services for Participants in relation to their identified needs.

Monitor authorized services through face-to-face in-home visits, phone calls, person centered planning team meetings, and contact with service providers.

The Service Coordinator position is performed remotely from home and will require intermittent travel within Bucks, Chester, Delaware, Montgomery and Philadelphia counties.


Competencies:

Education/Experience:

Must have a Bachelor's degree in social work, psychology or other related fields with practicum experience.

OR

In lieu of a Bachelor’s degree, must have at least three (3) years’ experience in a social service or healthcare related setting.- Bachelor's degree in Social Work or related field preferred.
