General Summary:
Under the supervision of the Program Manager – AI, the In Home/Community Support Specialist is responsible for implementing the In Home and Community Supports or IHCS service by assisting and supporting participants with the skill building activities needed to maintain and increase independence/inclusion in both home and community settings.
Responsibilities:
1.
Implement individual service needs which promote independence and the achievement of individual outcomes as stated in Individual Support Plan (ISP).
2.
Develops, plans and implements goal/skill driven programs/activities for
the individual.
3.
Prepares and completes all forms of documentation as required.
4.
Demonstrates commitment to customer service to fulfill agency mission
and values.
5.
Provides personal care assistance as necessary in a safe and consistent
manner consistent with guidelines stated in the individual support plan in order to facilitate the individual’s ability to participate in program activities.
