1. Develops, implements and evaluates individual service needs of program participants which promote independence and the achievement of Individual outcomes as defined in the individual ISP under the supervision of the Program Manager. (40%)
2. Responsible for the planning, development and implementation of the program activities. (10%)
3. Completes and prepares all forms of documentation as required. (40%) 4. Provides personal care assistance as necessary in a safe and consistent manner. (5%)
5. Adheres to agency wide and departmental specific policies and work rules to ensure efficiency and compliance (5%)
A master’s degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities
OR
A bachelor’s degree and 2 years of work experience working directly with persons with disabilities
An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities.
ADDITIONAL EXPERIENCE:
Previous experience in the administration of an adult day program or adult recreational setting preferred. One-year supervisory experience plus experience working with people with physical and other disabilities preferred.
ADDITIONAL QUALIFICATIONS:
Excellent communication skills in both written and spoken language. Effective organizational skills. Ability to prioritize and manage multiple tasks simultaneously. Computer skills including word processing and data base applications helpful. Due to meeting schedules and participant needs, must be able to accommodate travel to remote locations, which are not located near public transportation. Valid driver’s license.
