General Summary:
Under the supervision of the Program Manager – AE, the Community Support Professional is responsible for implementing the Community Participation Support service by assisting program attendees to increase involvement and inclusion within their communities through desired recreation, independence and skill building activities.
Responsibilities:
1. Implements individual service needs to promote independence and the achievement of individual outcomes as stated in the Individual Support Plan (ISP) through community integration.
2. Develops, plans and implements programs that are engaging to individuals attending the AE program.
3. Prepares and completes all forms of documentation as required.
4. Demonstrates commitment to customer service to fulfill agency mission and values.
5. Provides personal care assistance as necessary in a safe and consistent manner consistent with guidelines stated in the individual support plan in order to facilitate the individual’s ability to participate in program activities.
6. Adheres to agency-wide and program-specific policies and work rules to ensure efficiency and compliance, including but not limited to: (5%)
