Responsibilities:
• Development of the person centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals
• Coordinate services for Participants in relation to their identified needs
• Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providers

The Service Coordinator position can be performed remotely and will require travel within Lycoming and Clinton Counties. 

Competencies:

Education/Experience:

  • Registered Nurse OR

  • Have a bachelor’s degree in Social Work, Psychology, or related fields with practicum experience OR 

  • Have at least three (3) years’ experience in social service or health care related setting
