General Summary:

Under the supervision of the Program Manager AE, the Community Support Professional is responsible for implementing the Community Participation Support service by assisting program attendees to increase involvement and inclusion within their communities through desired recreation, independence and skill building activities. 

 



Responsibilities:

 

1. Implements individual service needs to promote independence and the achievement of individual outcomes as stated in the Individual Support Plan (ISP) through community integration. (70%)

 

a) Works on a 1:1 or small group (1:3) basis, supporting growth in areas of life

     skills development and independence.

b) Provides information, support and role modeling to individuals to increase

 their ability to self-advocate, develop community networks, participate in

 community activities, and to achieve levels of independence.

c)  Is responsible for the daily care, transportation, training and supervision of individuals.

 

2. Develops, plans and implements programs that are engaging to individuals attending the AE program. (15%)

 

a) Collaboratively works with individuals and other AE team members to plan and establish social, recreational, skill building and volunteer opportunities that meet the individuals’ wants and needs.  

 

3. Prepares and completes all forms of documentation as required. (5%)

 

a)  Completes documentation or arranges for the completion of documentation as required, regarding individual outcomes as defined in each individual support plan.  This includes documentation necessary to maintain licensure.

      b)  Participates in the development of the individual support plans

    c)  Utilizes SAGE Time and Attendance and SAGE Employee Self Service

  System to comply with payroll and benefits requirements. 

 

4. Demonstrates commitment to customer service to fulfill agency mission and values. (5%)

 

  1. Completes additional departmental tasks as assigned.
  2. Responds to all verbal and written inquiries from individuals, the individual’s family members, outside agency staff, other outside contacts and UDSF staff in a prompt and courteous manner.

c)  Displays courteous and professional decorum in all interactions.

d)  Manages information in a confidential and professional manner at all times.

e)  Greets and interacts with the individuals in a pleasant and reassure manner.

f)    Maintains a neat appearance to represent personal image in a positive and professional way.

 

 

 
5. Provides personal care assistance as necessary in a safe and consistent manner consistent with guidelines stated in the individual support plan in order to facilitate the individuals ability to participate in program activities.  (5%)

 

a)  Assists with toileting needs.

b)  Assists with ambulation and wheelchair maneuvering, including safely

     assisting individuals on/off a van lift.

       c)  Feeds or assists individuals with eating.

                   d)  Provides other personal care as requested by the individual.



Competencies:

Education/Experience:
 

High school diploma or GED. 

 

 Minimum of one year experience working with people with disabilities, plus experience in an adult day program or adult recreational setting preferred.
