Under supervision of the HCBS Program Specialist for Learning and Development and in combination with the participant, the Transition Service Coordinator (TSC) will arrange and ensure participant services in accordance with the assigned program requirements.  


1. Travels to counties within the UDS business footprint to conduct five (5) day visit in order to assess and develop the person centered service plan.  Obtains plan approval of plan from either Office of Long Term Living (OLTL) or the Managed Care Organization (MCO).

2. Maintains and effectively manages a caseload of participants to be determined by Program Specialist until such time as they can be transitioned to other Service Coordinators. This includes identifying and monitoring the services provided to participants through OLTL/BAS/MCOs and applicable regulations to determine quality of services provided and verify whether the type, amount, scope, duration and/or frequency of services need to be adjusted.

2.  In partnership with the participant, supports an individual’s goals to live in the most integrated community setting as independently as possible and to establish natural community connections.

3.  Completes all necessary forms enabling participants to continue to receive services in the time frame established by program guidelines and with the Agency’s established standards.

4. Maintains information required in HCSIS, SAMS, HHA Exchange, SC Database, Eligibility Verification System (EVS), HALO, eLTSS, and Envolve, systems to ensure accurate and timely information.




Must be an RN or have a Bachelor's Degree in social work, human services, psychology, sociology, anthropology, gerontology, behavioral health or other related fields

Must have at least three (3) years of experience in a social/human service or healthcare related setting

Fluency in English and Spanish is a plus.
