A Service Coordinator’s role is to support and advocate for Participants in need of waiver services thereby ensuring their health and safety as they remain living at home.


The Service Coordinator’s responsibilities are:
• Development of the person centered service plan that address Participants’ identified areas of need, SMART goals, intervention, and progress of goals
• Locate resources and supports for Participants enrolled in waiver services
• Coordinate services for Participants in relation to their identified needs
• Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providers
• Assist Participants in maintaining their health and safety through accessing member benefits in their home and community

• Reducing falls
• Identifying Emotional Wellness concerns
• Complaints and Grievances
• Evaluating Participant Experience

The Service Coordinator position can be performed remotely and will require travel within Philadelphia County.






Must have a Bachelor's Degree in social work, human services, psychology, sociology, anthropology, gerontology, behavioral health or other related fields

Must have at least three (3) years of experience in a social/human service or healthcare related setting

Must have proven ability to work in a Microsoft suite environment

Fluency in English and Spanish is a plus.
